Name and address of the owner(s) of the LLC 
A name, address, and phone number for a contact person. This does not have to be provided to the state, and will not be public, but us and the registered agent have to know it
What the company will do. If selling products, please be as specific as possible
Do you have a US SSN or ITIN?
US-based phone number (US-based is best if you have one)
